Introdução: A Reforma Psiquiátrica oportunizou o surgimento de serviços de atenção substitutivos aos hospitais psiquiátricos que proporcionassem a inclusão com base na estratégia da Reabilitação Psicossocial. Os Hospitais de Custódia e Tratamento Psiquiátrico ainda preservam ações contrárias à Reforma, indicando a necessidade de mudanças. Objetivo: Identificar os processos de trabalho desenvolvidos pela equipe de um desses hospitais e construir uma reflexão crítica acerca das possíveis contribuições da terapia ocupacional na instituição. Método: Estudo de abordagem qualitativa desenvolvido por meio de entrevistas com 11 profissionais de nível superior analisadas com a técnica de Análise Temática. Resultados: A falta de comunicação e do entendimento da necessidade do trabalho em equipe provoca ações individualizadas e desarticulação nos cuidados, evidenciando ações comprometidas com as necessidades institucionais e não com as necessidades dos pacientes; as falas destacam a falta de atividades voltadas à reinserção e participação social dos internos na família e comunidade; apesar de os profissionais afirmarem conhecer o conceito de Reabilitação Psicossocial, muitos atribuem um sentido equivocado, indicando-o como uma abordagem do modelo asilar que visa ocupar os internos para diminuir o ócio institucional. Conclusão: A articulação entre os profissionais do local é frágil, o que impede que as ações sejam realizadas de forma interdisciplinar e intersetorial, mantendo condutas prioritariamente no modelo asilar. A terapia ocupacional pode atuar como protagonista de mudanças, contribuindo para o alcance de objetivos condizentes com a Reforma Psiquiátrica, facilitando processos de cuidado alicerçados na Reabilitação Psicossocial.
